Rx for busy doctors: medical transcriptionists

"I love what I do for a living," says Gerry Kelly, who runs All Type, a medical-transcription business, from her home in East Brunswick, New Jersey. Kelly is one of approximately 9,100 members of the American Association of Medical Transcription (AAMT), many of whom are home based. No matter what happesn to the health-care system, the field of medical transcription appears to be headed for strong growth in the 1990s.

At an AAMT conference last year, Robert Love, a Houston attorney specializing in the health-care industry, cited his projection that the industry will expand by 100 percent by the year 2000. In his opinion, barely one-third of the hospital market is being tapped. In addition, health-care facilities don't always have the space or the budget to keep all their transcription work on-site. The money you can earn as a medical transcriptionist, depending on the time you devote, your client base, and your location, ranges from $25,000 to $40,000 a year, according to most reports. Kelly herself made $43,000 last year.

Doctors dictate audiotapes describing patient care, operations, autopsies, and lab reports. Transcriptionists use a transcribing machine to listen to the tapes through headphones; they input the information to a computer. Doctors and hospitals need the records both to ensure consistent patient care and to protect themselves against malpractice suits. The transcribed documents become permanent records of patient care.

Typing skills are important for medical transcribing, but competence is not measured only in words per minute. "Medical transcription is a language specialty, not a keyboard specialty," says Pat Forbis, a certified medical transcriptionist and director of member services at AAMT. "Your speed and accuracy are determined by how well you know the language doctors use."

The main drawback to medical transcription is that it demands intense mental and physical concentration. Transcriptionists are hooked to headphones listening to doctors' hastily dictated notes for hours on end; when hospitals require quick turnaround, they may work well into the night. And after years of intensive typing, they run the risk of suffering a repetitive motion disorder.

Florida's South Broward Hospital District and international law firm Greenberg Traurig LLP announced today they have brought a class action suit against MedQuist (MEDQ.PK), a publicly-traded company based in Mt. Laurel, New Jersey and the world's #1 vendor for electronic medical transcription services, following an independent investigation of MedQuist's billing procedures.

Delisted from NASDAQ June 16, MedQuist identified accounting errors following the complaints of an employee. In its public statements, in late July, the company detailed the results of an independent audit by Debevoise & Plimpton LLP and Pricewaterhouse Coopers which found some hospital clients were overbilled for transcription services. As a result of the investigation, MedQuist replaced their CEO and fired their chief financial officer and chief legal officer. The Securities & Exchange Commission has also launched an investigation of the company.

SSI Advisors, a management consulting company headquartered in New York, spent almost a year for one large hospital system evaluating MedQuist's over billing problem. Based on invoices reviewed, SSI Advisors' principal, Jeffrey Litvack, found through their analysis that the MedQuist over bill rates reached as high as 50% on some invoices and estimated a loss exceeding ten (10) million dollars to the hospital system.

The class action has been filed in the United States Central District of California by the international law firm Greenberg Traurig LLP who charges that MedQuist "knowingly and fraudulently overcharged for transcription services."

"MedQuist's fraud and over billing was intentional and that the fraudulent invoicing was designed specifically so that it was nearly impossible for the customers to trace the actual costs of the services provided," said Mark Hogge of Greenberg Traurig's Washington, D.C. office.


U.S. Spending on Medical Transcription Outsourcing Services Will Reach $4.2 Billion in 2008, IDC Study Reveals

The medical transcription outsourcing market has had to weather financial scandals, medical privacy threats, and offshoring trends that have threatened to decimate the U.S. market. Oddly, none of these factors has been sufficiently lethal to wreck or even weaken the transcription outsourcing industry that has grown around this important back-office task, mainly because it is crucial to the healthcare industry. In fact, new research published by IDC reveals that U.S. spending on medical transcription outsourcing services totaled approximately $2.3 billion in 2004. IDC forecasts that the market will increase to $4.2 billion in 2008, with a five-year compound annual growth rate (CAGR) of 16.1%.

"Medical transcription outsourcing is not as high profile, or even headline demanding, as IT needs around other healthcare issues such as the electronic medical record," stated Scott Tiazkun, program manager for IDC's U.S. IT Opportunity, Healthcare research. "What is important to remember is that information from medical transcriptions will be valuable in populating many fields of information in electronic medical records, and in some cases today acts as a de facto medical record."

IDC's research shows that although much medical transcription is still being performed with antiquated technology, IT vendors are springing up to provide this business process with new technology and more efficient and accurate ways to deliver a quality product to healthcare providers. A side benefit will also be reduced medical error rates.

Among the key findings presented in this forecast document are the following:

--Medical transcription outsourcing in the United States will remain in high demand due to new initiatives around the electronic patient record, as well as HIPAA regulations that demand that certain health records do not make their way overseas.

--Although there is some consolidation among larger players, the medical transcription outsourcing market is so overpopulated with smaller providers that consolidation is practically meaningless. Therefore, IT vendors need to make IT solutions and technology available at a price point that makes sense to technology companies that are the equivalent of mom-and-pop operations.

Transcription consolidation

This year has witnessed two mergers involving leading transcription service companies. Earlier this year, Webmedx Inc. acquired Atlanta-based Rodeer Systems. In the most recent deal, MedQuist Inc., the nation's leading provider of medical transcription services with $405 million in annual revenues, has acquired Lanier Healthcare, another company providing a broad mix of Web-based transcription and dictation services, electronic signature and speech recognition solutions, The new venture combines the forces of MedQuist's 9,800 transcriptionists serving 3,200 healthcare clients with Lanier's 480 employees and its established customer base of more than 285,000 physicians, estimated to be 50 percent of the acute care hospital market.

WebMD And MedQuist Enter Into Cross-Promotion and Integration Agreement

MedQuist to become exclusive provider of medical dictation and

WebMD, the leading full-service healthcare Internet site, and MedQuist Inc. MedicaLogic's enhanced transcription interface slated for release

In forming a strategic relationship with Marlton, N.J.-based MedQiust Inc. recently, MedicaLogic has shown its ongoing intent to make the electronic medical record (EMR) even more responsive to the day-to-day needs of the ambulatory physician practice.

Under the agreement signed in December -- the first EMR vendor partnership for MedQuist -- the companies are developing an interface between MedicaLogic's Logician(R) product and MedQuist's MTS(TM) transcription software, giving physicians the ability to have dictated information more easily incorporated in their patients' electronic records. The firms will market the solution to their respective customers and prospects, beginning with initial demonstration of it at the 1997 Health Information Management Systems Society (HIMSS) conference in February. General availability is slated for the second quarter of this year.

The MedQuist interface will be the latest addition to the suite of connections between Logician and other information systems.
